The process:

  1. To get started, please fill out our contact form or call (262) 240-9860.
  2. A representative from 65 Incorporated answer any questions you have and direct you to complete a questionnaire which helps our team better understand your unique needs.
  3. Once the questionnaire is completed, 65 Incorporated will contact you again to schedule a time for your consultation. You will then receive an email confirning your call in or webinar log in information.
  4. Please set aside 60 minutes for your consultation. (90 minutes for a couple)
  5. After the consultation, you will receive a link to download your personalized Medicare RoadMap report (provided in a PDF format) within 5 business days.
